Adults bolus IV :
20% IV x 3 doses (0, 4 and 8 hours)
40% IV (0, 1, 4 and 7 hours)
Continuous IV :
20% IV pre-chemo, 50-100% with chemo, then 25-50% x 12 hours
Children :
60-160 % of Oxazaphosphorine dose.
Dosage Adjustment :
Renal Impairment : Administer with caution.
Hepatic Impairment : Administer with caution. |
