ADULT DOSE :
4.0 to 6.0 mg/kg/day as TID / BID
PAEDIATRIC DOSE : 6.0 to 7.5 mg/kg/day.
Renally Impaired Patients : The initial dose is the same as that recommended for
patients with normal renal function.
