The recommended dose of oxaliplatin injection as single agent is 130/mg/m², given intravenously over at least 2 hours, every three-four weeks. In case of combination the recommended dose schedule given every two weeks is as follows :

Day 1 : Oxaliplatin 85 mg/m² IV infusion in 250-500 ml of Dextrose 5% and Leucovorin 200 mg/m² IV infusion in Dextrose 5% both given over 120 minutes.

Day 2 : Leucovorin 200 mg/m² IV infusion over 120 minutes, followed by Fluorouracil 400 mg/m² IV bolus given over 2-4 minutes, followed by Fluorouracil 600 mg/m² IV infusion in 500 ml Dextrose 5% (recommended) as a 22-hour continuous infusion.

Renal Impairment : Required in severe impairment.
